When the adapter is used on a stroller that also has a wheeled board accessory attached for transporting a standing toddler, and the car seat is positioned so the child faces forward, the car seat can disconnect from the adapter and fall.

In February 1996, CPSC issued guidelines (pdf) (which were incorporated into an industry voluntary standard in 1997) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.
Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard, or return the garment to either the place of purchase or to NTD Apparel for a full refund.

Creative Expressions has recalled about 43,100 paper party hats. The foil fringe at the bottom of the hat can detach and pose a choking hazard to children.
The recalled blue party hats have “1st” printed on the front and blue fringe glued to the bottom. They are cone-shaped and measure 6.5 inches tall and 4.5 inches wide. The hats were sold at party and discount department stores nationwide from February 2005 to May 2007 for about $4.

About 40,000 cribs from Simplicity Inc. are being recalled because of entrapment and choking hazards. Incorrect assembly instructions packaged with the cribs can cause the drop side to disengage and trap the child. The metal locking pins on the drop side can also pop off, posing a choking hazard.
The recalled cribs are part of the Nursery-in-a-Box furniture set along with a changing table and clothing organizer. They come in cherry, white and natural colors. The recall includes model numbers 8910 and 8050 with serial numbers 3005 HY through 0806 HY, which can be found on an envelope permanently attached to the mattress support and a label on the bottom rail of the headboard. The cribs were sold at department stores and children’s product stores from August 2005 to May 2007 for about $200.

Simplicity
The assembly instructions provided with the cribs incorrectly instruct consumers how to attach the crib’s drop side. If improperly installed, the drop side can disengage from the crib, posing fall and entrapment hazards for the child. Additionally, the metal locking pins on the drop side can pop off, presenting a choking hazard.
Consumers should immediately check the crib to make sure the drop side is securely fastened and correctly installed. Contact Simplicity to receive correct assembly instructions, or consumers can download the assembly instructions at the firm’s Web site, www.simplicityforchildren.com. Consumers can view a video on the firm’s Web site showing the proper assembly of the drop side. If the drop side is not properly installed, consumers should stop using the crib until it is assembled correctly.
CPSC is aware of an incident in which the crib’s drop side, which was installed upside down, fell from its upright position and the metal locking pins became dislodged. Simplicity received a report of wrong instructions being packaged with the crib.

Fisher-Price has recalled about 112,000 swings. Infants can get caught between the frame and the seat of the swing, resulting in cuts, bumps, bruises, and red marks.
The recalled Portable Rainforest Take Along Swings are about 23 inches high. They have a palm tree mobile and two plush toys hanging from the overhead bar, as well as two handles on the sides. The model numbers K7203, K7192 and K7195 are included in the recall (these can be found under the right handle of the swing). The swings were sold at discount department stores and toy stores nationwide from November 2006 to May 2007 for about $65.
